Home health services are provided by nurses, physical, occupational, and speech therapists, as well as dietitians, medical social workers and home health aides. These services can help avoid unnecessary hospitalizations, accelerate hospital discharges, and enable an individual to remain at home during acute illnesses.

Short-term home health care services are prescribed by a physician to help with rehabilitation after an illness, injury, hospital stay, or surgery, or to help manage a chronic medical condition with the goal of preventing an unnecessary hospitalization. This care is provided by nurses and physical, occupational, and speech therapists, as well as dietitians, medical social workers and home health aides through one-hour home visits and continues until recovery goals have been met. Episodes of care typically last between 30 and 60 days, but can be renewed if medically necessary.
